Contact & Booking Forms

We collect information such as your name, email and the nature of your dog’s condition on the contact and booking forms on our website. Before sending these forms you are required to agree to the storage and handling of your data by this website.

Vet Referral Forms

Canine Whispers can take Vet referred cases. In our Behaviour Consultation section you will find a button to download a Vet Referral Form to be filled out and posted in order to give us more information about your dog. Such information is then stored securely and used solely for the purpose of assessing your dog prior to your initial consultation. By posting the form to us you are consenting for us to store said information and use it for this purpose.
